A printed billing and shipping address. 2. Signature from an authorized member of organization that is responsible for making purchases for, or on behalf of, submitting organization. 3. A valid purchase order number. Please do not submit Requisition Numbers unless they are accompanied by a Purchase Order. 4. You may fax, email, mail or attach a copy of an purchase order to your online order. Your order will NOT ship until validation of your payment has been made All purchase orders must be paid within 30 days of invoice date in order to avoid any late fees or additional charges. Please contact us within 7 days of receipt of an order to file a claim or dispute any charges. - Paypal  You must have a PayPal account in order to use this as a form of payment.
